Emergency Certificates are issued only in cases of genuine emergency.
Requirements:
- Complete Form PP1 ( Fill Two copies)
- Submit the above form with the following:
- Two current passport photographs 1″ by 1.5″ (NB: Computer generated pictures are not acceptable)
- Expired passport.
- Two copies of Birth Certificate or National ID card.
- Letter of request explaining the emergency & a copy
- Home and work telephone numbers
- Two copies of Proof of emergency
- There is no fee for this service.
NB: Any incomplete application will be returned.
